Please Note: Inline with C.A.P.S. insurance all members must be over 4 years old in order to participate in C.A.P.S. Clubs / Camps / Activities / Events.

Proof of Diagnosis

Acceptable documents include: the CAPS Diagnosis Form signed by the relevant department, AsIAm Card, front page or letter from a clinician confirming the diagnosis of autism this is inline with our insurance so unfortunately the registration process can not be completed without it.

Annual Registration Fee: €20 per family

Forms may be returned by email or post and the annual registration fee (€20 per family) may be made via bank transfer, by post (cheques or postal orders only please) or alternatively in person.
